Update to Disinter/Reinter Form - Vital Records Wants Your Input
Greetings from the State Office of Vital Records,
We are preparing a new application and permit for the disinterment and reinterment of human remains. Once implemented, this application and permit will largely replace the current court order requirement to disinter a body buried in Georgia.
A draft of the new Disinter and Reinter of Human Remains form has been attached for comment purposes only. Please review the form, and send any remarks or suggestions to your Regional Consultant by 2/26/2021.
Instructions, trainings, and more details on the implementation of the new form will be provided after the comment period has ended. Please look for future e-mail announcements from the State Office of Vital Records regarding this matter.

A regional consultant serves as the liaison between partners/stakeholders and the State Office. Your assigned regional consultant (RC) can assist you with answering questions, resolving escalations.
